PRELIMINARY

Wells Operations
What happened While moving a 4,500 full tote
tank, the boom hoist cable on the crane parted
allowing the boom to fall uncontrolled across the
rig catwalk, riser deck, and mud tanks. Platform
ICS was initiated and rig operations were
suspended. A Roustabout (rigger) sustained minor
injuries when struck by falling wire rope from
the parted boom hoist cable. He was subsequently
medically assessed and released for return to
full duty. Operations are currently underway to
secure the boom and related equipment until
disassembly/replacement can be performed safely.
This bulletin is very preliminary and the
investigation continues. Additional transferable
learnings will be communicated as possible and
warranted.

  • Boom Hoist Cable had been visually inspected
    pre-use by the crane operator.
  • Cable was scheduled for replacement and was
    approximately 1 month beyond preventive
    maintenance (PM) replacement interval of 12
    months.
  • Engineering to review PM schedule and assess
    feasibility of replacing the wire rope based upon
    actual usage in addition to time.
  • Buffer zone under boom/load was adhered to
    resulting in minimal exposure to personnel on the
    platform.
  • Injured employee was not directly under the boom,
    but was struck by whipping cable.
  • Stay out from under crane boom/loads!
