ROE- District Contract

• Provider will be Maidez Center
• Districts will have a contract for each different
  building
• Maidez Center will enter contracts into website
• Contract number will be RCDTS code plus 4 digits
• Contract description will be school name plus ROE
  number
• ROE may view contracts on website
• Districts will be billed monthly based on actual
  hours of instruction delivered by Maidez Program

Compensation

• All Instructors
• Paid for the class taught at 5.00 per hour per
  student based on actual minutes of attendance
• Lead Instructor
• Paid for additional duties at .17 per student
  per hour of instruction delivered in program at
  school site based on actual minutes of attendance
  .17 x ( students x possible session hours)
  missed session hours
• Example .17 x (30 x 30) 2.75 152.53
• The program can only pay for actual minutes of
  instruction per SES guidelines.
• If a student is tardy, leaves early or misses a
  session and does not make it up, the teacher will
  not be paid for that time

Student Grouping

• Students will be placed in classes based on their
  Afterschool Achievers pre-test scores
• Class names will be assigned using the following
  format
• Grade level - Subject-Instructor last name
• Example 5 Reading Smith
• (If two instructors have same last name, add
  first name initials as needed)

Ordering Materials

• Lead Instructor places order based on class
  groupings.
• Once the order shipment is received, the Lead
  Instructor must confirm receipt
