Required Paperwork
  • CST REFERRAL PROCEDURES WHEN TEACHER REFERS 
  • Teacher is concerned about a students progress.
  • Teacher accesses building IRS team to generate a
    series of interventions/strategies to assist
    student toward success in classroom.
  • Building Principal forwards Referral to the Child
    Study Team form and copy of notice of referral
    letter to Special Services Office.
  • Director of Special Services sends notice of
    referral letter (form) to parents with enclosures
    (PRISE and SPSS).
  • Referral to the Child Study Team is stamped date
    received and logged in.
  • Child Study Team schedules Evaluation Plan
    Meeting within 20 calendar days of date the
    referral was received, and sends written notice
    of the Evaluation Plan Meeting to parents.
  • Evaluation Plan Meeting takes place. In
    attendance the three members of the child study
    team, the general education teacher, and the
    parent(s). If parents do not attend, the meeting
    needs to take place as scheduled, with an attempt
    to obtain parental participation by
    teleconference which is documented in the pupil
    record. The teacher brings to this meeting all
    pertinent data about the students educational
    performance. At this meeting, a decision is
    made whether to proceed with a child study team
    evaluation, not to evaluate, or to refer the
    teacher to the building IRS team for classroom
    interventions or possibly for a 504 Accommodation
    Plan.
  • If the decision is to proceed with a child study
    team evaluation, the specific assessments to be
    done are determined and written parental consent
    requested by the child study team.
  • Upon completion of evaluation, the child study
    team schedules an Eligibility Conference and, if
    appropriate, IEP Conference, with prior written
    notice to parents. In attendance the child
    study team case manager, the regular education
    teacher, the special education teacher, the
    parent (the IEP team). The parent must receive
    copies of the evaluation reports ten days prior
    to the Eligibility Meeting.
  • Note Written parental consent is required for
    child study team evaluation and for initial
    placement in a special education program.
