  • 1960-1970 - Deinstitutionalization unfulfilled
    promise of community care
  • 1970-1990 funding cuts but new models of care -
    AB377 Childrens System of Care and AB 3777
    Integrated Service Agencies
  • 1990-2000 Realignment, EPSDT, Parity and AB
    34/ AB 2034
  • 2001-2004 - Campaign for Prop 63
  • 2005- 2020 fulfill the promise

Fail first to help first
  • Hard to get help before failures such as
    hospital, jail, special education, out of home
    placement
  • New paradigm get the right help (client
    centered recovery oriented comprehensive
    community mental healthcare) at the right time
    (first sign of serious problems) at the right
    place - close to home

  • Close Holes in SED and SMI SOC Services
  • Invest in Prevention and Early Intervention

Be Patient and Attentive
  • No commitments without approved plans
  • State develops guidelines for plans and programs
  • Counties develop plans probably not all at once
    in first year
  • Plans reviewed locally and by the state
  • Funds allocated probably some programs in some
    counties before others

Protecting Current Funding
  • A county may not withdraw other mental health
    funding
  • Establish current allocations of funds in each
    county from all sources
  • Findings required under realignment for any
    significant change
  • State can limit 63 funds based upon other
    available funds

Expanding Programs
  • Hope on the Street is Expected
  • Expand existing SOC programs
  • Re-establish AB 34 outreach teams
  • Homeless discharges from jail, juvenile
    justice, hospitals, child welfare
  • Keep expanding until everyone is served
  • Reserve funds for new programs and planning

Train Staff for SOC
  • Only services that meet SOC standards in 5806
    (adults and seniors) and 5868 (children) can be
    funded under 63
  • County and provider staff must be retrained
  • 63 HR funds can pay for training
  • Academic programs need SOC curriculum

Prevention and Early Intervention
  • Most Important and challenging program
  • Need savings to ensure adequate funding
  • Not even allowed under Realignment
  • Mostly education and outreach not services
  • A core mission of mental health associations

  • Take time to get it right early pilots
  • Priority age group 15-25
  • Early Psychosis Programs serve as model -
    expanded to cover all mental illnesses and
    suicide prevention
  • Each setting and age group requires a different
    set of strategies

  • K-12 Schools
  • Higher Education
  • Workplaces
  • Clergy and other helpers
  • Oversight and Accountability Commission
