Whats been happening?

• Restarted the group in May 05 across East
  Lancashire
• Well attended spurred on by Preferred Provider
  Scheme
• Did a Path in July helped to give direction
  over 12 month period
• Hold meetings approx every month

The Aims

• To help people do their own plans eg service
  users, family members etc
• To help providers to plan with people to better
  get the life they want
• To ensure that planning with individuals helps to
  plan for better services and supports now in
  the future
• To best work out how we spread and implement
  person centred planning and approaches

What needs doing?

• Help people to learn how best to develop their
  own plans
• Help providers to learn how to best help
  individuals what makes sense!
• Showing that person centred planning makes a
  difference to peoples lives
• Make sure that we know how many people need help
  with planning
• Making sure training is available to all
• Make sure that in East Lancs we have lots of
  people who can deliver training

Whats been done so far?

• All providers have a named PCP Champion
• Exploring the use of the website for individuals
  to plan and as somewhere to look for guidance
• Raised awareness of linking HAPs PCPs
• Supported Person Centred Reviews in Transition
• Communication in PCP
• Mapping out future demand
• Mapping out a training plan
• Identified the need for a PCP Coordinator
• Worked toward families taking a lead with
  planning

What Next?

• Regular Core Group
• Working out how best to involve family carers and
  service users
• Follow the Path
• Identify a regular Chair
• Gain the approval of the Partnership Board to
  appoint a PCP Coordinator
• Firm up plans for training
• Make sure people who need plans get them
• Develop the website
• Offer training for people who want to use the
  website to plan

Now For The Really Important Bit!!

• To get Managers from all Organisations to be
  committed to Person Centred Planning and make
  sure it really makes a difference.
• This includes Support Providers, Connexions,
  Care Management Teams, People from Childrens
  Services etc etc
