These indicators require identification of the
patients diagnosis. If medical claims data or
clinical information are not available for
determining the diagnosis, a drug-based algorithm
may be used to identify patients with heart
failure e.g., identify patients with concurrent
use of 1) ACEI or ARB 2) loop diuretic and 3)
beta-blocker.

A list of contraindicated medications will be
developed for each measure based upon the ACC/AHA
guidelines for management of chronic heart
failure, and other available evidence.
