Breakthrough Series Aims

• Reduce the rate of anticoagulant adverse drug
  events per 1000 doses by 50 on 2E by May 2005
  (or use a risk measure as a surrogate)
• Double the number of surgical cases between
  surgical site infections (SSI's) in a pilot
  population
• Offer patients a same day appointment with their
  Primary Care Provider or team member in their
  Primary Care Provider's absence
