    Check Request Form (found here on ASCMC.org)
  • Attach the receipt(s) and proof of advertisement,
    if relevant
  • Turn the form into the person who oversees the
    fund from which the money comes from (e.g. Isayas
    for Senate funding, Josh for club funding, etc.)
  • For special circumstances, contact that person
