FORM 1
  • Use the current version (9/2005) upper left
    corner
  • FROM is the District name
  • Watch the project start and end dates
  • Fiscal Agent and Indirect Cost rate must be
    approved
  • Signatures in blue ink must be dated
  • Summary and Detail budgets must match
  • Title funds on left portion of Detail budget
  • Transfer funds on right portion of Detail budget
  • Break out the private/charter school expenses
  • Watch the Indirect Cost codes and calculations
  • NO GRANT FUNDS MAY BE EXPENDED WITHOUT FORM 2

  • All required pages must be submitted
  • Submit only one set

Cover Page
  • District Name this must be completed!
  •     
  • Districts planning to use funds in programs to
    begin July 1, 2007 will need to submit their
    application by June 1, 2007. You must provide the
    DOE with at least 30 days to review/approve an
    application.

  • Subsequent changes to documentation should
    be sent to each Title program.

Private/Charter Schools
  • Private/Charter School Participation
  • Form is Required the eligible school list will
    be posted to the DOE website when allocations are
    available
  • Form is to be Completed and Signed by the
    Private/Charter School
  • Private, Nonprofit Schools must indicate they
    have submitted the proof of their nonprofit
    status to the NHDOE

    district activities OR receive equitable services
  • There are two check boxes to distinguish the type
    of participation

Affirmation of Consultation
  • Retain This Form in the Program File
  • DO NOT SEND IT TO THE DOE
  • How were the childrens needs identified?   
  •     What services are to be offered?   
  •    What service delivery mechanisms were
  • considered?
  • How, where, and by whom will the services be
  • provided?   
  •    Describe the size and scope of the equitable
  • services to be provided to the eligible
    private school
  • children, teachers, and other educational
  • personnel.

  •   Describe the consultation. What decisions
    were
  • made about the delivery of services?
  • What were the private schools views on the
  • provision of services?  
  •    The proportion of funds that will be allocated
    for the
  • nonpublic school services.
  • The form must be signed and dated by the
  • Private/Charter School
    official.

No Reply
  • When private/charter schools do not reply, send
    evidence to indicate that the school received the
    request to participate but did not respond. A
    Certified Mail Return Receipt is the accepted
    audit paperwork.

Charter Schools
  • All charter schools in NH are public schools but
    they are not an LEA.

Local Transferability Act (FLEX) (Section 6123)
  • Transfers for LEAs that are not in Title I
    Corrective Action or Title I School Improvement
    (50 limit)
  • Dollars may be Transferred Out of
  • Title II-A, Teacher Quality - Section 2121
  • Title IV, Safe and Drug-Free - Section 4112(b)(1)
  • Title V, Innovative Programs - Section 5112(a)
  • NOTE private schools cannot transfer funds
    unless the LEA does.
  •

Transferability FLEX continued
  • Transfers for LEAs that are in Title I Corrective
    Action or Title I School Improvement may only
    transfer 30 of their funds
  • Funds may only be used for school improvement
    activities under sections 1003, School
    Improvement and 1116(c),academic assessments and
    local agency and school improvement

Section 1116 - Improvement
  • School Improvement Title I, Section 1116
    activities include
  • LEA must incorporate scientifically based
    learning in core academic subjects
  • Identify actions that have the greatest
    likelihood of improving student achievement
  • Address professional development needs of
    instructional staff

  • Include specific measurable objectives for each
    group of students identified in the disaggregated
    data
  • Address fundamental teaching and learning needs
    in the schools and specific academic problems of
    low-achieving students
  • Incorporate, as appropriate, activities before,
    after, and during the summer
  • Incorporate strategies to promote effective
    parental involvement in the school

REAP Transfer
  • For the flexibility provision of REAP (SRSA)
  • Districts must have a National Council of
    Education Statistics Locale Code of 7 or 8
  • Have fewer than 600 students enrolled
  • Districts may transfer 100 of funds
  • List of eligible districts can be found on the
    DOE website

Carryover
  • Beginning in FY2008 (July 1, 2007) there will be
    no carryover for
  • Titles IIA, IVA, and V

GEPA 427General Educational Provisions Act
  • Section 427 requires each applicant for funds to
    include in its application a description of the
    steps the applicant proposes to take to ensure
    equitable access to and participation in, its
    federally assisted program for students,
    teachers, and other program beneficiaries with
    special needs
  • Must be filed and accepted by the Department for
    the 2007-2008 year
