Click green LOG IN button

Click the Self Registration button

Administrative tab

Click Recommended Information

Complete all required information and optional
information (if available)

Click to look up your State

Type in the name of your state
Click OK

Click on your states code

Click to look up your Organization

Select applicable information
Site name is the name of your command (Check with
DTA)

Once you have found your organization, click
Select
