OF PQDRs
Category I - May cause death, injury, or severe
occupational illness loss or major damage to a
weapon system critically restricts combat
readiness capabilities or which would result in
a production line stoppage. Category II - Does
not meet criteria set forth in category I.

Request Form and follow directions. A password is
not necessary but allows you to check PQDR
status after submission). To submit a deficiency

DODAAC, (NOT UIC), then 03 for the year
2003 and a sequence number you assign. For
