Process Details
  • The process took a quite few months it helped
    that we were in the process of updating our
    Nursing History Form and we decided to add the
    questions to the form.
  • Adding this section was a part of the overall
    process.

Directions to nursing staff
  • Section C Addictions
  • Any tobacco users in the house refers to other
    than the child.
  • A referral to the Smokers Helpline may be for
    the parent and/or the child.
  • Document any information given
  • Upon discharge referrals are to be faxed to
    Smokers Helpline

Nursing History
  • Required documentation upon admission- less
    likely to be missed during assessment.
  • The questions are asked in relation to family
    members as well as patient.
  • The faxing of the referral was really dictated by
    the Helpline requirements that it be done upon
    discharge.
  • We ask that the ward clerk fax the forms when
    they dismantle the chart at discharge.

Feedback
  • The nurses think its good to have the questions
    on the form
  • Do not always ask the questions
  • Will not ask the teenager in front of parents as
    they feel it is less likely to get a truthful
    answer
  • Opens up dialogue positive
  • Resistance?

Orientation to Smokers Helpline
  • Very helpful
  • Information on the need and support offered to
    smokers once referred
  • Gave direction to how to offer the materials
  • Gave us materials to offer pamphlets, posters,
    referral forms

Recommendations
  • Do the information sessions first
  • Managers unit staff
  • Easier if everyone knows what the program entails
    its objectives
  • Decide what questions you really need to ask when
    best to ask them and what format the information
    seeking will take
  • Make sure written information is readily
    available.
  • Intensive in-service then follow-up
