Current and Future Locations

  • Ten years of grass roots community efforts by
    Advocates for a Healthy Community, Inc., led to
    the establishment of JVCHC in 2002.
  • JVCHC delivers quality primary medical and dental
    care to residents in Greene County, the third
    largest community in Missouri.
  • JVCHC was awarded the 2002 Salute to Health Care
    Award by the Springfield Area Chamber of
    Commerce.
  • Currently, services are provided at two
    locations, a dental clinic at 618 N. Benton and a
    medical clinic at 3000 E. Division.
  • An additional medical clinic at 630 W. Kearney
    will open in April 2005.

Aim
  • Jordan Valley will redesign the clinical
    practice of our patients with diabetes by
    implementing the Care and Improvement Models in
    order to develop strategies that improve health
    outcomes and eliminate health disparities.

Population of Focus
  • During the Collaborative, Jordan Valley will
    focus on the patients of one of our providers
    (Dr. Bentley). This includes approximately 213
    patients with diabetes who have been identified
    as active clinic patients of Dr. Bentley.

Key Diabetes Measures

Registry
  • JVCHC utilized VirtualCHC to host PECS on our
    server.
  • Populating our registry is a collaborative effort
    between providers, nurses, medical records,
    support staff, and information systems (Key
    contact Stella Plaster).
  • 30 patients are currently entered on our registry.
