Definition

• Systematic analysis of in-patients deaths and
  complications (adverse events) by the medical
  staff
• To flag possible failures in patient care
  (avoidable event?)
• To identify and correct the causes or factors
  which contributed to the failures

History

• Activity developed in the beginning of the 20th
  century in the USA as a teaching tool
• Mandatory in the course of surgeons initial
  training
• Then, extended to other specialities and other
  countries
• Based on the principle of learning by the error
• The recognition and the analysis of the medical
  errors is a powerful tool for improvement of
  personal skills

  and safety of care.
