• Access AI/AN Web-site www.ou.edu/hiv
• Click on Data-Entry link.

• Select Submit local data for Washington.
• User Name database
• Password apips
• Username and password are case sensitive
• Use Internet Explorer
• Remember to close your Internet Browser after
  submitting all of your data.

Practice Accessing Forms

• Launch Internet Explorer.
• Go to AI/AN webpage.
• Click on Data Entry link.
• Submit local data for
• Choose Washington.
• Enter username and password.
• You will be given 3 data submission method
  choices.
• Print a paper form.

Practice E-Forms

• Print a paper form if you havent done so already
  and complete it.
• Access the project data entry page on the web.
• Choose Adobe PDF and transfer information from
  the paper form to the interactive PDF.
• Submit
• Repeat previous step using HTML forms.

Paper Forms - Plan B

• If you ever need to submit faxed or mailed forms,
  please keep these points in mind
• CALL ME!!!
• Forms must be first-generation copies
• Handwriting must legible and not touch edges of
  boxes
• Bubbles must be filled in completely
• Sets of forms must remain together
