Objectives

• Create a one to two page Quiz/Test Template
• Insert Page Numbers
• Insert your name in the footer
• Insert Placeholders for
• Title
• Questions 10 questions
• Answers 4 answers per question
• Save completed document as a Word Template (.dot)

Insert Page Numbers

• Click Insert on the Menu Bar
• Select Page Numbers

Bottom of Page
Right
Click OK

Insert Header/Footer

• Click View on the Menu Bar
• Select Header and Footer
• Type Created by Your Name in the Footer
• Center Created by Your Name

Click Close

Insert Title Field 1

• Click Insert on the Menu Bar
• Select Field

Type Insert Quiz or Test Title Here
Select MacroButton
Click OK

Enter Question Field 1

• Leave a blank line between the title field and
  the first question
• Select Numbering on the toolbar

Enter Question Field 2

• Follow the steps on slides 4 5 to create a
  field for your first question
• For Display Text enter Type Question 1 Here
• Hold down the Shift key on the keyboard and press
  the Enter key

Turn on the Forms Toolbar

• Click View on the Menu Bar
• Select Toolbars
• Select Forms

Do Not Use Protect Form on your template
Check Box
Form Field Shading
Protect Form

Enter Answer Fields

• Select a Check Box form field
• Press the space bar once
• Follow steps on slides 4 5 to insert an answer
  field
• Hold down the Shift key on the keyboard and press
  the Enter key
• Repeat steps 1-4 until you have 4 answer fields
  for your 1st question

Tips

• You may copy and paste so you dont have to
  create individual fields just be sure to right
  click on the fields, select edit field, and
  rename the Display Text
• After you enter the last answer field for
  question 5 hold down the Ctrl key and press
  enter to insert a page break so that question 6
  begins on page two

Saving Your Template

• Click File on the Menu Bar
• Select Save As
• Select Document Template (.dot) for save as type
• Change save in to your www folder
• Click Save
