    with a list of authorized students to be offered
    service and their assigned provider
  • 9. Complex areas may send authorization letters
    for parents via the school
  • 10.Schools send authorization letter to parent,
    authorizing services by the assigned provider.
    Letter includes the provider's contact
    information

    up Agreement of Services meeting

    meet to develop and sign the Agreement of
    Services (AOS)

    on-site observations and/or desk audits of SES
