For Cause Drug/Alcohol Testing -Non-DOT
Supervisor contacts OMS that an employee may
have ETOH/drug problem
Is this for Cause?
OMS advises as appropriate
NO
OMS Program Manager will notify American
Diagnostic Laboratories, Inc. that the supervisor
is bringing an employee for testing
After hours, supervisors should call
301-706-5239, identify themselves as MC employee,
give employees name and say which site employee
will be tested and arrange for a staff person
from American Diagnostic Laboratories, Inc. to
meet him/her at the lab.

• OMS Instructs Supervisor
• Give copy of AP 4-11 to employee
• Obtain employees permission to obtain
• specimen and to release information
• Fax signed copy to OMS 240-777-5132
• OMS holds for MRO review report
• Accompany employee to American
• Diagnostic Laboratories, Inc. facility for test
• 6. Arrange safe transport home for employee

Test results positive?
Note Results of breath alcohol testing are given
immediately

OMS can phone Supervisor to inform
OMS submits written report of results to
Departmental Director or her/his
designee indicated in writing
MRO interviews employee to confirm
Note Urine drug and breath alcohol testing are
done at the American Diagnostic Laboratories,
Inc. facility in Silver Spring during regular
business hours as well as after business hours.
