Scholarship

• Congratulations! By being invited to this
  meeting, you have met the scholarship requirement
  for the NHS!
• No further documentation of scholarship is
  necessary
• Make sure to continue to keep your GPA above a
  3.500

Service

• Please refer to the Service Template in your
  packet.
• Up to 10 service activities total (combination of
  in-school AND out-of-school)
• All activities MUST be from high school career
• Quality/depth of service is important
• For activity to count, you must have the
  position, contact or e-mail, and signature of a
  sponsor

Leadership

• Please refer to the Leadership Template in your
  packet
• Up to 10 leadership activities total (combination
  of in-school AND out-of-school)
• Be specific regarding leadership duties and
  responsibilities
• Leadership and service descriptions for the same
  activity should be different
• Leadership qualities will also be shown in
  recommendation forms
• For activity to count, you must have the
  position, contact or e-mail, and signature of a
  sponsor

Character

• According to the NHS, the student of good
  character upholds principles of morality and
  ethics, is cooperative, demonstrates high
  standards of honesty and reliability, and shows
  courtesy, concern, and respect for others.
• Based largely on disciplinary record
• Character qualities will also be shown in
  recommendation forms from school/community
  sponsors

Recommendation Forms

• Please refer to the NHS Membership Application
  Recommendation Form in your packet
• Must obtain 3 recommendation forms (with
  signatures) to be considered
• 1 from teacher
• 1 from community sponsor
• 1 from applicants choice
• Do not procrastinate in giving forms to
  teachers/sponsors
• Sponsors will put forms in sealed envelopes, sign
  the seal, and give them back to the applicant,
  who will then turn them in with the application
  packet

OPTIONAL Personal Statement

• If you wish, you may submit a personal statement
  of no more than 100 words with your application
• The purpose of this statement is to show
  qualities you believe you possess that would be
  an asset to the NHS
• See packet for format requirements
• Choosing not to complete a personal statement
  will in no way affect the outcome of the
  decision-making process by the Faculty Council

• Do not put packet in a binder or report cover
• Staple the pages in the following order
• Cover Page
• Service Template
• Leadership Template
• Recommendation Forms (3)-in envelopes
• Personal Statement (OPTIONAL)
• Signature Page
• All documents needed to assemble your packet can
  be found on the NHS tab on the LHS website
• You can type directly into these documents
