  • Even with technical advances, cataract surgery is
    still a concern in monoculars.
  • Both patients and ophthalmic surgeons may often
    prefer to delay the treatment until the cataract
    progresses to more advanced stage.
  • Purpose To evaluate the outcomes of cataract
  • surgery in monocular patients retrospectively.

Methods
  • Records of 71 monocular patients who had
    cataract surgery with phacoemulsification by a
    single experienced surgeon were reviewed.
  • The data analyzed for each patient includes
  • - Demographic information - Anesthesia
    method
  • - Pre- and postoperative VA - Additional
    surgery
  • - Co-morbidities in the operated eye - IOL
    model
  • - Reason for poor VA in the unoperated eye
  • - Intra- and postoperative complications
  • Clinical data were expressed as mean and
    percentages.

  • Gender (F/M) 32/39
  • Mean age 72.3 yrs (27-91)
  • Mean follow-up 16 wks (4-78)
  • Cause of monocularity
  • 14 previous ocular surgeries.
  • 86 preexisting ocular pathologies.
  • Anesthesia 73 topical 11 subconjunctival
  • 14 subtenon 1 general
  • Implanted IOLs Hydrophobic acrylic.

  • Complication
  • No capsular tear
  • PCO 12.6
  • Endophthalmitis 1.4
  • Ocular co-morbidity 68 (46 eyes).
  • 23 AMD 17 DR 4 uveitis
  • 6 glaucoma 7 PEX 3 maculopathy
  • 6 corneal pathology
  • VA 92 pts had an increase in VA at least
  • 2 Snellen lines.

Conclusion
  • Although satisfactory, cataract surgery in
    monocular patients are likely to have some ocular
    co-morbidities and risks, some of which may limit
    the final visual outcome.
  • Surgeons skill and experience are essential for
    surgical outcomes as well as considering all risk
    factors and obeying the surgical principles.
