Title: Failsafe

1
Failsafe
  • Better safe than sorry

2
Definition of 'fail-safe'
A fail-safe device in a nuclear weapon to
deactivate it automatically in the event of
accident"
  • 1. (noun) fail-safe a mechanism capable of
returning to a safe state in case there is a
    failure or malfunction
  • 2. (adj) fail-safe guaranteed not to fail
  • 3. (adj) fail-safe eliminating danger by
compensating automatically for a failure or
    malfunction

3
Fail-safe operation
  • 1. Operation that ensures that a failure of
equipment, process, or system does not propagate
beyond the immediate environs of the failing
    entity
  • 2. A control operation or function that prevents
    improper system functioning or catastrophic
degradation in the event of circuit malfunction
    or operator error.

4
Back to Basics
  • An Industrial building is to be equipped with a
fire alarm system, activated by any one of
    several latching switches installed throughout
    the facility.
  • Simple right??
  • But What if

5
A Failsafe approach..
  • The same problem can be solved this way..

Only difference.
The goal of fail-safe design is to make a control
system as tolerant as possible to likely wiring
or component failures.
6
Why be Fail-safe
  • Chernobyl Incident
  • A group of single-minded but non-nuclear
engineers directing a team of dedicated but
    over-confident operators. Each group probably
assumed that the other knew what it was doing.
And both parties had little or no understanding
of the dangers they were courting, or of the
    system they were abusing
  • Errors and Procedural violations resulted into a
    tragedy

7
The impact
Massive OR Impressive?
8
HISTORY
  • Failsafe systems, which control potentially
hazardous operations to ensure they are shut down
if faults occur, have in the past traditionally
    been hard-wired installations.
  • Serious drawbacks of hard-wiring approach
  • An inherent inflexibility
  • Lack of data communications facilities

9
We Step in..
  • These are prompting a growing move towards using
specialized systems based on programmable logic
controllers (PLCs) that are tailor-made for
    failsafe applications.
