    successful?
  • What social skills do they bring?
  • What jobs or work environments should be avoided?

you have explored what it could lead to!

8
Person Centered Planning (PCP)What is it?
  • A personal vision of what an individual would
    like to be and do
  • Emphasizes preferences, talents, dreams ( not
    needs/limitations)
  • Takes advantage of existing supports
  • Involves family and friends in planning and
    action steps

9
Why do a PCP?
  • Empowers the individual to drive the process of
    goal achievement
  • Helps support people share the responsibility of
    being involved in action steps
  • Brings stakeholders together to work as a team

10
How do you do a PCP?
  • Individual decides who he/she would like to be
    involved in the meeting
  • One of the group members organizes a time and
    place for everyone to meet
  • Facilitator writes down notes and plan using a
    PCP form
  • Each person leaves with a task or tasks to
    accomplish
  • Tentative meeting to re-evaluate plan

11
Person-Centered Employment Planning
  • Allows for self-determination
  • Focuses on ability rather than disability
  • Builds confidence and ability to promote self
  • Uses existing relationships for support
  • - Friends, family, community members
  • Increases engagement, job satisfaction, and job
    performance

12
30-Day Placement Plan What is it?
  • A month long plan towards finding employment
  • Includes

13
Why Write One?
  • Most agencies require some sort of plan for
    employment
  • Usually they are long-term
  • Typically they are broad and do not offer step by
    step instructions
  • Can focus on the services the agency will provide
    instead of action steps

14
Who is involved
  • Any job seeker
  • Their Support Network (providers, friends,
    family, neighbors, etc.)
  • Family, Friends
  • Residential staff, other professionals
  • Employment Specialist/Job Developer

    getting a job

    involved in the job search
  • Sense of accomplishment, even if a job was not
    found yet

16
When Do You Write One?
  • At one of the first meetings to work on goals
  • Every 30 days until person is comfortable in
    their new position

    support needs
