Chemical Mediators of Inflammation
  • Include
  • vasoactive amines eg histamine and serotonin
  • neuropeptides
  • plasma proteases
  • arachidonic acid metabolites ie prostaglandins,
    leukotrienes and lipoxins

Arachidonic Acid (AA)metabolites
  • Collectively called eicosanoids
  • Act like short-acting, local hormones
  • Have roles in almost all parts of inflammatory
    process
  • AA is a component of cell membrane phospholipids,
    metabolised by phospholipase
  • Two main metabolic pathways, 1 results in
    prostaglandins, 2 in leukotrienes

Reduction of Inflammation
  • Blocking phospholipase (corticosteroids see next
    lecture)
  • Blocking the prostaglandin pathway
  • Note there are 2 forms of cyclo-oxygenase (COX1
    and 2), which is the key enzyme involved in PG
    synthesis.
  • COX 1 is found in gastric mucosa where it is
    important in maintaining stomach mucosal
    defences. It also occurs in most other tissues
  • COX 2, discovered in 1990, found in synovial
    cells and macrophages
