Overview

• Purpose To build a collaborative community
  effort supporting youth with disabilities to
  transition from school to community living.
• Community partners will assist in the
  development of resources and supports and engage
  youth in their skills-building and access to
  tools for successful transition.
• Commit resources of partners and others to
  implement successful youth transition programs in
  the Coastal Bend.
