Incident Management Process
Responsibility any vendor that experiences
an incident while working for Shell.

• Report to Shell immediately
• Immediately quantified as within 24 hours,
  irrespective of day or hour
• Property Damage, Spill Overboard, or an Injury
  report immediately
• Basic Information who, what, when, where, how
  bad does it look

• 3 Business Days
• Supporting / Additional information beyond that
  provided in the initial report
• Immediate causal factors that should be addressed
  or shared

• 30 Days
• Complete investigation package
• Facts of the incident
• Immediate Causes
• Systemic Causes
• Corrective Actions
