NM-DROPPED OBJECT- CRANE SLING FALLS TO GROUND
What happened While prepping to move a well
stand, the crane operator raised the main block
while lowering the fastline ball with two slings
attached. No load was attached at the time. As
the fast line ball passed the main block, one of
the slings came into contact with the main block
and rotated the hook. This allowed the fast line
hook to orientate towards the main block. The
sling then unlatched from the fast line hook,
dropping the ½ inch sling roughly twenty feet to
the ground. No personnel were injured. All
personnel were outside the established buffer
zone of the crane. The crane operator lowered the
fast line ball, and found that the safety keeper
was not properly connected.
- Preliminary Learning Points

- JSA had been reviewed for lifting and hoisting
operations during the start of the morning shift.
During the job, 3-4 hours had lapsed in between
lifts in which the JSA was not reviewed before
starting the lifting operation again. Only a
pre-job was held. - Two single slings were attached to the fast line
and four single slings were secured to the main
block to eliminate re-hooking the slings to each
block during the job. - Securing pin for the safety latch on the fast
line was not inserted in the correct attachment
point. - Both fast line and the main block line were being
moved at the same time. As per OPS 0055, this is
not an approved practice unless it is a complex
lift.

Development Procedure to ensure that HSE Early
Learning Bulletins are developed
correctly. Distribution When the Learning
bulletin is completed send it to your HSE
Supervisor, the supervisor will review and
forward to the HSE Team Lead for the region for
final review. HSE Team Lead will submit to the
ELB focal point for distribution. Filing The
ELB focal point will file distributed learning
bulletins in the EPW RTD ELB- Livelink.
