her grade level (K-11) for the upcoming school
year. Also, to indicate registration for
Sacramental preparation, please check all boxes
that apply. Sacraments require two-year
preparation beginning in 2nd grade
(Reconciliation and Eucharist) and 10th grade
(Confirmation) or later as necessary.
Please Note For any Sacramental Preparation,
please provide the parish office with a copy of
your childs Baptismal Certificate.
