  Navy Healthcare system is to support the
  healthcare needs of the Navy and Marine Corps
• Each and every Sailor and Marine is important
• We must have healthy Sailors and Marines so the
  mission can be accomplished

• The mission of the Navy and Marine Corps is
  harmed by tobacco use
• Tobacco users
• Have more lost work time
• Utilize healthcare more often
• Have more sick days
• There is no safe form of tobacco

• We as military and civilian members of the Navy
  Healthcare Team must set the example
• Tobacco use by a Navy healthcare team member
  defeats the message of tobacco harm
• Tobacco use by a Navy healthcare team member
  defeats the message of Leadership by Example

The process of tobacco cessation

• The process of tobacco cessation for our Sailors,
  Marines, and other eligible beneficiaries is very
  easy to remember- it is the 5 As
• Ask
• Everyone is asked about tobacco use
• Everyone using tobacco is asked about quitting
• Advise
• A simple positive, empowering message encouraging
  the patient to quit
• Assess
• What factors about tobacco are harming the
  patient?
• Is it just money? Health? Time? Family?
• Assist
• Get the patient the help they need for quitting
• Know what resources are available
• Arrange
• Everyone wanting to quit should have their needs
  addressed
• Help should be convenient

How to address tobacco in the clinical setting
(Ask)

• First- everyone is asked Do You Use Tobacco
• DO NOT say do you smoke because this will miss
  those who use smokeless tobacco
• Ask about tobacco at every available healthcare
  setting
• If the answer is
• Yes then ask Do You Want to Quit (over 60 of
  tobacco users want to quit)
• No then congratulate the non-tobacco user for
  making a good healthy decision
• No, I quit the ask how the quitting process is
  progressing and if the quitter needs any help and
  let them know that they can come in any time to
  receive help if needed

The response.

• When asked Do you Want to Quit, the person
  using tobacco can answer in three ways
• No I dont want to quit
• Ill think about it or Im not sure at this
  time
• Yes, I want to quit

Your response to..

• No I dont want to quit
• This response is not met by threats or scare
  tactics
• Simply tell the user that if they ever decide to
  quit that they can return at any time to get help
• If they have tried to quit before let them know
  that new medications and techniques are available
  that may work better than before
• Ill think about it
• Let the patient know that cessation help is
  immediately available
• Let them know that medications and support are
  available
• Get them to make a best guess on a date when they
  might wish to quit
• Yes, I want to quit
• Congratulate the patient
• Get the cessation help started

A strong message (Advise)

• Everyone using tobacco should receive a strong
  message that
• Quitting tobacco is the most important health
  choice you can make
• Quitting tobacco will make give you more
  spending money
• Quitting tobacco will set a great example for
  your family and friends
• Quitting tobacco will help your children be
  healthy

The next steps for cessation (Assist)

• Know your resources
• Have the cessation process clearly understood
• Who do you pass the patient off to?
• Do you help them right away?
• Who is in charge of tobacco cessation in your
  spaces?
• Handing off the patient to a distant location
  does not help
• Less than 5 of those who want to quit will call
  or visit another location
• The teachable moment cannot be transferred to
  someone else

The final step is Arrange

• Every patient should be able to receive tobacco
  cessation when it is convenient for them
• Some will only need a pamphlet, some only
  medications, and some may need the classroom
  setting
• Arrange an initial cessation session while the
  patient is in your spaces
• Find out what they feel they need for cessation
  support
• Work with your tobacco cessation team to develop
  clear pathways for those wanting to quit
