Service Related RightsI HAVE THE RIGHT TO
TERMINATE (STOP) OR REFUSE SERVICES.

• I understand that I need to do the following as I
  exercise this right.
• To go to the boss and explain why I want to
  stop or refuse services.
• Remember that people may not agree with me
  because by doing so I may get sick or harmed.
• Remember that I could lose my services and not
  get them back easily

I HAVE THE RIGHT TO KNOW, IN ADVANCE, SERVICE
LIMITS.

• To learn about what services I can and cannot get

CHARGES, WHETHER INSURANCE, GOVERNMENT FUNDING,
OR OTHER SOURCES COVER SERVICES. I HAVE THE
RIGHT TO BE NOTIFIED UPON REQUEST OF CHANGES IN
THOSE CHARGES AND BE TOLD OF ANY CHARGES I OR
OTHER PRIVATE PARTIES MAY HAVE TO PAY.

FROM INDIVIDUALS WHO ARE TRAINED AND COMPETENT,

AS REQUIRED, AND WHO MEET ADDITIONAL
QUALIFICATIONS IDENTIFIED IN MY INDIVIDUAL
SERVICE PLAN.

• To know that staff are trained to help and
  support me.
• To have understanding and helpful staff.

Protection Related Rights
I HAVE THE RIGHT TO PRIVATE RECORDS (PERSONAL,
FINANCIAL, SERVICES, AND MEDICAL INFORMATION) AND
KNOW THE LICENSE HOLDERS POLICIES AND PROCEDURES
REGARDING DISCLOSURE OF SUCH INFORMATION. I HAVE
A RIGHT TO SEE MY RECORDS.

• I understand that I need to do the following as I
  exercise this right.
• I need to know what staff writes or puts in my
  records.
• I need to remember to return the records, after
  reading them, in the same shape that I got them
  in. I cant tear them up if I dont like what
  they say.

I HAVE THE RIGHT TO BE FREE FROM MALTREATMENT.

• I understand that I need to do the following as I
  exercise this right.
• I need to know that maltreatment means bad
  treatment.
• I will treat people how I want them to treat me.

I HAVE THE RIGHT TO BE TREATED WITH RESPECT AND
COURTESY FOR MY OWN INDIVIDUALITY, THE WAY I
COMMUNICATE, CULTURE AND RECEIVE RESPECTFUL
TREATMENT OF MY PROPERTY.

• I understand that people should treat me with
  respect.
• I think that people should respect my own
  personal property.
• I understand that people should respect my
  culture/heritage as well as the way I
  communicate.

HAVE THE RIGHT TO ANY OF THE LICENSE HOLDERS
PROCEDURES FOR GRIEVANCE OR COMPLAINT RESOLUTION
AND THE RIGHT TO APPEAL UNDER SECTION 256.045. I
HAVE THE RIGHT TO KNOW THE NAME AND ADDRESS OF
THE STATE, COUNTY, OR ADVOCACY AGENCY TO CONTACT
FOR ADDITIONAL INFORMATION OR ASSISTANCE.

• I understand that I need to do the following as I
  exercise this right.
• I need to know who to tell, how to reach them and
  how to ask for help.
• I understand that no one will hurt me if I tell
  on him or her.

I HAVE THE RIGHT TO HAVE MY PROBLEMS RESOLVED.

• I understand that I can call my parents and ask
  for help.
• I can go to my social worker.
• I can talk to the staff.
• I can call an advocate.

PERSONALLY OR HAVE THEM ASSERTED BY MY FAMILY OR
LEGAL REPRESENATIVE, WITH OUT RETALIATION. I HAVE
THE RIGHT TO ADDITIONAL ASSISTANCE.

• I can call 911.
• I can take it to the court and appeal what they
  said or did.
• I can talk to another staff member.
• I can talk to the boss or the supervisor.

• I can learn about my rights from another
  self-advocate.

CONSENT TO PARTICIPATE IN ANY RESEARCH OR AN
EXPERIMENTAL TREATMENT.

• Ask staff to inform me and define for me all the
  terms and conditions before I agree.
• Get good information before making a decision.

I HAVE THE RIGHT TO HAVE DAILY, PRIVATE ACCESS TO
AND USE OF A NON-COIN OPERATED TELEPHONE FOR
LOCAL CALLS AND LONG-DISTANCE CALLS MADE COLLECT
OR PAID FOR BY ME.

  I exercise this right.
• Remember that sometimes I need to wait until a
  person is done before I can use the phone.
• Remember that I may have to share the phone if
  necessary.
• Remember that I need to pay my phone bill
  (including any long distance charges).

• Know that staff cant tell me who my friends are
  or who I can be friends with.
• Remember that I have to be nice and flexible.
• I should reschedule if I forgot to go somewhere
  with someone.

• I should tell people when I want to be alone.
• Close the door for privacy when I want to be
  alone.
• I can tell people to go away if I want to be
