• Recruit 200 practices of lt20 PCPs in late 2009.
• Practices randomized by CMS into Study Group or
  Control Group.
• Study Group eligible for incentives (additional

• Year 1 Adoption of CCHIT-certified EHR
• Year 2 Reporting of quality measures
• Year 3-5 Clinical improvements (P4P)

Practice Requirements

• Size
• Small to medium-sized practices (lt 20)
• NPs/PAs as well as MDs/DOs
• At least 50 Medicare Fee For Service (FFS)
  beneficiaries
• Specialty
• Primary care - Family Practice, General Practice,
  Internal Medicine, Gerontology
• Medical sub-specialists only if practice is
  predominantly primary care
• Note May or may not have EHR at time of
  application

Minimum Required EHR Functionalities

• By end of 2nd year, Demo practices must utilize
  EHRs to perform minimum functionalities
• Patient visit notes
• Recording of lab/diagnostic test orders results
• Recording of prescriptions

Incentive Payments

• Two separate per-beneficiary incentive payments
  offered
• HIT incentive payment for performance on Office
  Systems Survey (OSS)
• Quality incentive payment for reporting or
  performance on 26 clinical measures
• Diabetes
• Congestive Heart Failure
• Coronary Artery Disease
• Preventive Services
