Title: CLINICAL EVALUATION

1
CLINICAL EVALUATION
  • A Sample Of A Completed Evaluation Tool

2
PURPOSE OF EVALUATION
  • Allows students to evaluate strengths and
    weaknesses
  • Allows students to develop goals for their own
    learning
  • Assists students in demonstrating progression of
    learning
  • Provides feedback to improve knowledge base
  • Provides feedback to improve performance
  • Provides documentation to support student
    progression or lack thereof

3
EXAMPLEOF A COMPLETED CONCEPTS IN CLINICAL
NURSING I EVALUATION TOOL
4
OBJECTIVE I-The nursing student will utilize the
nursing process to organize and deliver safe,
effective care for clients various developmental
stages.Sub-objective 1
  • Collects data from the clients record and from
other health team members prior to providing
    care.
  • U-process tool not complete. Patient care
categories must be addressed for patient safety,
i.e.. how many to transfer, ambulate

5
Objective I Sub-objective 2
  • Compares collected data with established
    standards using the appropriate resources.
  • U-need to identify reason for abnormal lab values
as identified in the Labs Diagnostic book based
    on patient condition

6
Objective ISub-objective 9
  • Clusters and interprets data
    appropriately, including basic need and
    abnormal labs dx tests.
  • U-abnormal labs not identified as to why
    patients are abnormal

7
Objective ISub-objective 10
  • Identifies correct functional pattern based on
    assessment data
  • U-functional pattern selection should be based on
patients condition as indicated by chart review
and physical assessment-your patient was hypoxic
with an O2 sat of 80, your functional pattern
selection should have been identified as
    Activity/Exercise, not Health Maintenance

8
Objective ISub-objective 11
  • Ranks each functional pattern in order of
    priority.
  • U-Functional patterns must be ranked in order of
priority 1 or 2-next week make sure you rank

9
Objective ISub-objective 12
  • States appropriate desired outcomes or goals for
    client's problem/strength in each functional
    pattern.
  • U-Goals must be specific to patients cognitive,
physical, and developmental abilities. A patient
with dementia will not be able to design a
    brochure about the ADA diet

10
Objective ISub-objective 16
  • Interventions reflect assessments, cares,
  • treatments, and teaching.
  • U-Interventions did not reflect the cares
appropriate to this patient. You did not address
    ROM, ambulation, assistance with feeding
teaching importance of mobility and repositioning
to prevent skin breakdown and immobility
    complications.

11
Objective ISub-objective 17
  • Cites rationale and resources (including page
    numbers) to support the interventions
  • selected to assist the client in reaching
    their goals.
  • U-Text and page number not cited for each
    rationale.

12
Objective ISub-objective 20
  • Completes assigned care within an appropriate
    time frame.
  • U-Bath not completed within appropriate time, if
you are not going to be able to complete cares
you must notify the primary nurse and myself,
including information regarding why you are not
    able to complete

13
Objective ISub-objective 23
  • Revises goals and
  • interventions in response to the clients'
    changing condition.
  • U-Goals not revised as we discussed on Thursday,
that the goals needed to be more patient specific
    and measurable

14
Objective ISub-objective 27
  • Evaluation completed and written according to
    criteria.
  • U-Goals not evaluated on Friday as to why
    partially met or not met.

15
Objective II-The nursing student will effectively
communicate with the client, based on their
developmental stage, and health team
members.Sub-objective 10
  • Reports significant
  • information accurately
  • to other members of the
  • health team in a
  • professional concise
  • manner.
  • U-You did not report a change in your pts.
condition to the primary nurse. Any changes in
condition or abnormal findings, must be reported
to the primary nurse, and to me, so proper
    treatment can be initiated.

16
Objective IISub-objective 11
  • Verbalizes client
  • information from the nursing process tool in a
    concise manner before starting care.
  • U-Not able to verbalize pathophysiology of
disease, normal lab values, antidote to Coumadin,
what to do if pt. has hypoglycemic reaction.

17
Objective IISub-objective 12
  • Verbalizes pertinent information about
  • prescribed medications.
  • U-Not able to verbalize classification of MOM,
that it is a saline laxative, and that you dont
give to someone with undiagnosed abdominal pain.

18
Objective IISub-objective 13
  • Documents according to the policy of the
    clinical facility.
  • U-Student altered nurses notes after instructor
    approved documentation documented nurses notes
on physicians progress notes. Documentation must
not be altered once instructor approves.

19
Objective IISub-objective 14
  • Utilizes correct medical and nursing terminology
    and spells correctly.
  • U-Documentation is a legal document-spelling and
grammar must be correct use only facility
approved abbreviations (SOB is not approved here)
    per our conversation last week.

20
Objective III-The nursing student will
demonstrate professional behavior in fulfillment
of responsibility to self and others.
Sub-objective 1
  • Work on self-control-need to maintain control and
not raise voice or use sarcastic tone when given
    feedback on your performance. Although
constructive criticism does not always feel good
at the time, it is part of professional growth
and we learn from it. We continue to receive
feedback on our performance throughout our career
and it is not professional to raise your voice or
get sarcastic with the person giving you the
feedback. Take from it, evaluate its benefits to
you, and grow from it to become a better nurse.
Remember, it is not a personal assault. In the
future I expect you to remain calm and approach
me in a professional manner, or ask to take a
break to regroup. You shared with me that others
have expressed you raise your voice when given
constructive criticism, so I would encourage you
to be alert to cues this is happening and develop
    a technique to minimize.
  • Maintains professional standards of conduct at
    all times.

21
Objective IIISub-objective 2
  • Protects clients confidential status.
  • U-Must not release info about pt. to anyone who
does not have direct need to know to provide
care. I recognize you did not understand the
ramifications of releasing info about my sons
surgery to me However, this could have resulted
in a lawsuit under the circumstances. In the
future, this could result in a critical incident.

22
Objective IIISub-objective 5
  • Adheres to dress code as addressed in Nursing
    Student Handbook.
  • U-Per Nursing Student Handbook, only clear nail
polish is acceptable. On Thursday, you came to
clinical with bright pink polish. In the future,
any dress code violations will result in you
    being sent home.

23
Objective IIISub-objective 9
  • Maintains self-control.
  • U-See Objective III 1

24
Objective IIISub-objective 10
  • Completes documentation in the proper time
  • U-Per my instructions during orientation,
documentation is to be completed by assigned time
to facilitate staff access to the chart and prior
to physicians rounds. You did not complete your
documentation until 930am and it was to be
completed by 8am. In the future, you must notify
me by 745am if you are having difficulty
    completing your assessment/documentation.

INSTRUCTOR EVALUATION of STUDENT
  • Unsatisfactory Clinical Week
  • Objective I 14/2556
  • Objective II 10/1567
  • Objective III 7/1258
  • This week you did well with patient cares and
interaction. However, your med, lab, and disease
knowledge was not at the level expected for
Concepts I. In order to be successful next week

26
  • Know and be able to verbalize pathophysiology of
    diseases
  • Know and be able to verbalize classification,
pertinent side effects, and nursing implications
    for medications.
  • 3. Know and be able to verbalize norms for lab
values that apply to your patient
  • 4. Functional patterns and goals need to be
    specific to patient

27
  • Functional patterns must be ranked 1 and 2, in
    order of priority
  • Goals need to be measurable
  • Process tool must be complete when you arrive at
clinical Tuesday/ Thursday morning. If unable to
complete a section you must notify me when you
arrive with reason why area unable to be
    completed, i.e., not on chart
  • 8. Documentation must be completed by 8am. If
unable to complete you must notify me by 745am.

28
  • I understand that this is a difficult week for
you. You have shown great compassion to your
patients and have possessed a strong
knowledge-base in the previous 2 clinical weeks.
I trust that you will again demonstrate these
    next week.
