General Appearance

• Posture body flexed, hands clenched
• Skin Color consistent with ethnic background
• acrocyanosis, circumoral cyanosis,
• mottled when undressed
• observe for bruising
• texture smooth, soft, flexible may have dry,
  peeling hands and feet

General Appearance

• Turgor elastic
• Pigmentation milia across bridge of nose,
  forehead, or chin
• Erythema toxicum (newborn rash)
• Petechiae of head or neck r/t presentation

Head

• Fontanel slight pulsation, bulging w/ crying,
  stooling
• Shape of head molding
• 1/4th of body size
• 2cm gt chest
• Lacerations

Face

• Ears Moro reflex, alignment to ears
• Eyes bright clear, slight nystagmus, blue or
  slate blue gray, brown color at birth in dark
  skinned infants
• Nares patent sneeze common
• Mouth palate, suck, rooting, Epsteins pearls

Neck/Shoulder

• Palpate neck for mass
• Clavicles
• Neck appears short as chin rests on chest

Chest

• Symmetry
• Breast buds engorgement, liquid discharge, may
  be firm
• Lung sounds
• Heart sounds, PMI

Abdomen

• Bowel sounds, How soon?
• Umbilical cord, vessels
• Abdominal breathing

Pulses

• Femoral
• Inguinal
• Brachial

Genitalia

• Male centrally located meatus
• testes descended, rugae
• voiding?
• Female Labia majora-minora, clitoris
• Evaluate the size

Buttock/ Anus

• Symmetric
• Anus patent and passage of meconium within 24-48
  hours
• No fissures, tears, or shin tags

Extremities/Trunk

• Short generally flexed
• Extremities move symmetrically through ROM but
  lack full extension
• Good muscle tone
• Joints move spontaneously

Back

• C-shaped spine
• Flat straight
• At least half of back devoid of lanugo
• no dimpling
• Full-term infant in ventral suspension should
  hold head at 45 degree angle, back straight

Reflexes

• Blink
• Moro response to sudden movement or sound
• Rooting/sucking
• Palmar grasp
• Stepping
• Babinski Fanning extension of toes
