• HKCS contains items from
• Colorado Youth Survey (risk and protective
  factors)
• Youth Risk Behavior Survey (from the Centers for
  Disease Control)
• Asset and resiliency scales
• Supported and developed by many Colorado state
  agencies including
• Department of Education
• Department of Public Health and Environment
• Human Services (Alcohol and Drug Abuse Division)
• Public Safety (Division of Criminal Justice,
  Office of
• Adult and Juvenile Justice Assistance)

• High School Version 142 Items
• Middle School Version 127 Items (no sex or hard

  before administration date
• Mailing Includes
• Surveys, Scan Forms, Cover Letter, Teacher

  Envelopes, FedEx Return Labels
• OMNI will follow up with Admin Contact 2-3 weeks
  after administration if surveys not returned

HKCS Reporting

• OMNI will provide CPP Project Coordinators
  district level reports for participating
  districts in their county (unless otherwise
  specified in the MOA)

• Timelines
• If OMNI receives district surveys by Dec 30th,
  community will receive complete district reports
  by Feb 15th
• If OMNI receives district surveys by Oct 31st,
  community will receive draft of Substance Abuse
  section by Dec 30th
