4
AIM OF THE HFGE
  • TO PROMOTE AND INFLUENCE THE REDUCTION OF RISKS
TO AIRWORTHINESS RESULTING FROM HUMAN PERFORMANCE
    IN ENGINEERING AND MAINTENANCE

5
A STRATEGIC APPROACH
DATA
DESIGN
SENIOR INFLUENCE
ACTIVE ENGAGEMENT
INTERVENTION STRATEGIES
6
PROGRESS -DATA
  • Analysis of Wide Range of Reports and Surveys
  • Predominant Error Types
  • Installation Errors
  • Incorrect Installation
  • Incorrect Maintenance
  • Dominated by knowledge-based and rule-based
    error
  • Common Performance Shaping Factors
  • Pressure
  • Training
  • Fatigue
  • Value of Human Factors Programmes
  • Conclusions? Work in progress, but . Design
    ..Error Prevention .Error Recovery

7
PROGRESS - DESIGN
  • Discussion Paper
  • Suggested Best Practice
  • Training for Designers in Human Factors
  • Balancing Conflicting Requirements
  • HF Expertise in 2nd/3rd Tier Suppliers
  • Regulation not sufficiently definitive
  • Plenty of Ideas How to Take Forward?

8
PROGRESS SUCCESSFUL INTERVENTIONS
  • Methodology for Analysis
  • Data Dependent
  • Environment Specific?
  • More work to do.

9
PROGRESS INFLUENCE AND ENGAGEMENT
  • Position Paper
  • Common Message

10
We have come a long way, but challenges remain
  • Regulators focused on compliance rather than
    effective implementation
  • Regulators are not looking at the intent of the
    regulation
  • Implementation has been very training centric
  • And that training has focused on cost/compliance
  • and these organisations are not reaping the
benefits and are becoming increasingly frustrated!

11
Is this an organisation you know?
  • HF? We've done that
  • We published our HF procedures
  • We trained everyone in HF
  • We comply with all current regulations.
  • Our regulator audited us and it didnt result in
any findings regarding our HF programme
  • We fix problems as they occur

12
What does a healthy HFEM programme look like?
  • What are the Key Performance Indicators?
  • A healthy reporting culture indicates
  • Senior Management buy in
  • A real Just culture
  • Effective error investigation
  • Feedback system is working
  • Effective interventions
  • A reduction in error costs
  • If its monitored!

13
THE MESSAGE
  • We are heading in the right direction
  • Its about changing cultures and behaviours
  • Its about effectiveness not just compliance
  • Our success is measurable

its going to be a long journey ...
but it is a shared journey
14
WHAT NEXT?
  • Action Plan?
  • More Data Analysis
  • How to Influence Design
  • Small Operators, Small Manufacturers
  • Learning from Others
  • Influencing Senior Management
