Requirements

• NSF for OLDER People was required by 1st April
  2004 although
• most LA/Health have gone manual as interim
• Provision for Contact,Overview,Assessment
  Summary,Care Plan
• Design will include Generic template for ALL
  toolkits
• Facilities to inc authority own question set and
  database
• Built in logic and workflow protocols to assist
  functional rating
• Various models proposed to allow sharing and
  storing of data
• Key to success is the operational protocol for
  capturing data and
• agreeing roles of stakeholders
• SCH will automate SAP by December 2004

What is CareAssess?
A module of CareFirst,the software used in SCH
to manage client recording SCH have 3000
CareFirst users Other local LAs also use
CareFirst (Solihull, Warwickshire,
Telford,Shropshire,Northants) A major agenda item
is joint working. OLM working with SCH and
others to develop functionality for SAP (data
capture exchange)

How Does CareAssess Work?
CareAssess is a flexible tool for the creation of
electronic assessment forms integrated with the
existing client system (CareFirst) It allows SCH
to set up assessments with the questions used in
their local forms, including SAP forms Questions
can be pre-populated with data already held on
the client Assessments can be saved and viewed as
assessments. Data is saved back to base tables
for use elsewhere
