completes) the chart review

The team schedules activities for each team
member - kind of a role/responsibility. These
are simultaneous responsibilities so that when
the GWTG Patient Management Tool Participating
Hospital Agreement (PHA) is finally signed
everything else is completed also. As this
process progresses the team receives reports and
then the actual team meetings tend to be very
productive.
