• Mississippi is one of nine states chosen for the
  three-year CMS pilot program
• Must have a diagnosis of heart failure and/or
  diabetes
• Primary objectives
• Improve clinical quality
• Improve beneficiary and provider satisfaction
• Achieve overall net cost reduction of at least 5
• Develop new strategies for chronic care management
