MFP Grant Goals
  • GOAL 1
  • Ensure that all programs are considered when an
    individual decides to make the transition by
    educating and informing DHS staff and other
  • stakeholders
  • GOAL 2
  • Establish nursing facility transition workgroups
    at the local level to enable individuals to
    transition from the nursing facility to the
    community.

Implementation Plan
  • The MFP Grant initiative consists of two
    components
  • Component I
  • Educating DHS staff and stakeholders about all
    state/regional administered community care
    programs to ensure that all programs are
    considered when an individual decides to make the
    transition
  • Component II
  • Develop a local community approach to coordinate
    needed services, using the diverse expertise of
    local workgroups to transition NF residents
    interested in returning to the community

  • Component 1 Education
  • ?DHS developed a training curriculum of all
    state/regional administered community care
    programs.
  • The training curriculum includes a step by step
    guide on how to apply for services

  • DHS will deliver the training to DHS staff,
    providers, local advocates and other stakeholders
  • DHS will share training materials with workgroup
    members, advocates, providers, and other
    stakeholders
  • Add the training to the DHS website

  • DHS published an RFP to procure a contractor to
    establish nursing facility transition workgroups
    in every region.

  • Identify and recruit workgroup members
  • Develop policy and procedures for workgroups to
    conduct business
  • Provide training and technical assistance to the
    workgroup, relocation providers, DHS staff,
    advocates, and other stakeholders involved in the
    transition process

  • Participate, as needed, in person centered
    planning for individuals needing assistance with
    their transition
  • Ensure that clients supports and needs are met
    using a person-centered approach
  • Coordinate needed services and resources to
    ensure a successful transition

    all options are considered
  • Identify and address transition barriers and
    issues
