- Some actions have already been taken for you in
order to facilitate the exercise. - In order to keep the scenarios moving along we
are taking actions for some players that might
not be the action you would have taken. - Data Collection
- Please appoint a scribe to take notes for your
table. This will be essential to getting a good
after action report. - Please complete your Participant Evaluation Forms
before departing.

4Exercise Objectives
- Test the knowledge of the hospital staff
regarding the methods to prevent contamination of
the hospital. - Test the ability of the hospital staff to
recognize the need for and implement the incident
command system. - Understand the process that the hospital will
need to follow to obtain chemical antidotes from
the regional CHEMPACK caches. - Understand the process that the hospital will
need to follow to obtain laboratory blood
collection supplies to test for and identify
chemical contamination.

5Background
- YCO the local Youth Community Organization, is
in the process of preparing for a large sale of
antique farm equipment to raise funds for their
latest community project. The YCO is planning to
upgrade the walking trails surrounding the
community by adding environmentally friendly
solar lighting and other security enhancements.
The YCO has approximately 35 members aged 11-18.

- Mrs. Margaret Miller has decided to move
from the family farm to a residential community
closer to town due to the recent death of her
husband, Mitchell. Mrs. Miller and her daughter
Melissa have agreed to donate the contents of the
barn to the YCO for their farm sale in exchange
for the naming of a local trail after Mr. Miller.
Mr. Miller was a highly decorated WWII veteran.

6Background
- A call is received by the local 911 dispatch
from a highly agitated high school student
reporting that students and sponsors from the YCO
are very ill and requesting an ambulance.
Several have passed out and are shaking like
crazy Several others are nauseous and have
vomited. The caller reports that they are in the
process of cleaning out the barn at the Miller
place out on Route 4. Two ambulances have been
dispatched. The student is still on the phone
with dispatch.

- What additional questions should the 911
dispatcher ask of the student? -
- What concerns should be relayed to the EMS
personnel? -
- Who else, if anyone, should 911 notify of the
incident? -
- What actions, if any would the hospital be taking
if they heard the radio traffic between EMS and
911?
8EMS On Scene
EMS is on the scene and radios back that they
approx 20 students and 3 adults that are at the
scene. The students and 2 adults outside of the
barn seem groggy and their eyes are tearing. EMS
suspects that some chemical agent may be involved
and indicates that they are requesting assistance
from the fire department. They can see at least
3 students lying in the barn. One appears to be
having a seizure. There is also an adult at the
far end of the barn. They have been told that
there are maybe 3 or 4 kids in the loft. EMS
also reports that the adult sponsor says that two
of the high school kids got in their car and left
for the hospital already. EMS moves the
students outside the barn to a remote area upwind
and begins the triage process and the process of
gross decontamination. They also begin the
process of trying to account for each student.
They learn that there were 25 students and 3
sponsors cleaning out the barn in preparation for
a sale. Of the students they have triaged most
complain of blurred vision, shortness of breath,
some abdominal pain, and tearing of the eyes.

10Fire Department Arrives
The fire department arrives on the scene and are
told of the suspect chemical and immediately don
personal protective equipment and begin
appropriate processes such as ventilation of the
building, decontamination of students, rescue of
the injured/ill. EMS reports that they need
additional EMS assistance. Their triage report
is BLACK tags 3, RED tags 6, YELLOW tags
7, GREEN tags 8 Patients are enroute and have
been grossly decontaminated by the fire
department. The fire captain reports that he
has made contact with Mrs. Miller, the owner of
the barn. She is distraught to learn that
students are ill. She tells the fire captain
that her husband was a bomb specialist in the
war. She remembers that he cleaned out the barn
years ago and he got rid of some chemical
souvenirs. Mrs Miller tearfully reports that if
she had known that there was anything left in the
barn she never would have let the kids in to
salvage the farm equipment.

What procedures will the hospital use to obtain
chemical antidotes from the CHEMPACK stockpile?
What procedures are in place to protect staff
from chemical contamination by patients?
Please draw a rough diagram of the plan for
decontamination at the hospital.
12Laboratory Request
The fire department notifies the regional hazmat
team and local emergency management. Through
appropriate emergency management channels, the
CST is dispatched. The CST phones the hospital
to request information about the patients
symptoms and asks if the hospital has begun the
process of collecting the required chemical blood
and urine samples to send to KDHE/CDC for this
unknown exposure.

the stockpiled laboratory supplies to collect
blood and urine from the exposed patients? -
- How will the hospital assure that urine samples
get collected 6 hours after the initial exposure?
