• The medical cost for a replatation is three times
  that of an amputation.
• The number of days absent from work for a
  replantation of the thumb is half that of an
  amputation.
• The economy to society is of more than 40 000
  for the replanted thumb.
• It is the rate of functional sequelae which
  increases the overall cost of amputations (more
  75 of the total cost)
• Replantation of the thumb is an absolute
  indication

• First-web-space deepening will generally provide
  excellent results for amputations at the distal
  half of the middle third
• In the proximal half of the middle third,
  lengthening of the thumb ray is generally
  required

• The health insurance companies must not stop at
  medical costs of replantation.
• The social and economic impact is important
  because the replanted thumb ensures the return to
  work and decreases considerably the annuities.
• The organization of the emergency department of
  the hand is the best way to respond to the
  request of the populations.

Discussion

• Despite the elegant reconstructive options
  available, the best results are obtained with
  replantation or revascularization whenever
  possible.

• Distraction lengthening of the first metacarpal
  is a useful and reliable technique that provides
  up to 3 cm of length without requiring complex
  microsurgical methods
• Spare parts from another injured digit may be
  used in the acute setting for pollicization or
  heterotopic replantation.
