2Whats the SmartCard Program?
- The SmartCard Program rewards you for working
safely. How? When you work safely - Youll earn points redeemable for gifts and
prizes! - Youll receive chances at winning a Dream
Vacation!
3How Does It Work?
- Every month, following a safety meeting, your
supervisor will issue you a smart card. - Read and understand the safety lesson answer the
two T/F questions.
4Record Your Answers
- To record your answers, call the SmartCard
Hotline at 1-866-690-6345. - Youll be prompted to key in your clock and the
Quiz printed on the SmartCard. - Call in to record your answers in a timely manner!
5When you call the hotline
- Listen closely to the prompts and carefully key
in the required information to ensure accuracy. - After entering your clock number, listen
carefully when it is repeated back to you. This
number is used for recording award points and
grand prize entries. - If you enter the wrong clock number, you may not
receive credit for your award points and grand
prize entries or another individual may receive
the credit.
6You could be an instant winner!
- Every month, a random number of callers will hear
a message that they are an instant winner of
award points. - You can use your instant winner points
immediately by ordering a gift from the Award of
Excellence Catalog or save them up for a
higher-level gift. The points will be recorded
in a paperless account for you. - Even if you are not an instant winner, you will
earn a chance at a Dream Vacation just for
calling.
7Dont call more than once to record your answers!
- The SmartCard hotline will not allow you to
record your answers more than once to a
particular quiz. - You may call the Hotline any time to check your
point balance. These calls do not generate
points or entries for Dream Vacation trips. - Select the point query option when prompted.

9You Did It Right! Cards
- Your Supervisor will be watching to catch you in
the act of working safely. - If you are seen demonstrating one of the nine
behaviors printed on aYou Did It Right! Card, you
win. - Call the phone number printed on the card, choose
You Did It Right from the menu and enter the
PIN number printed on the card. - You may hear a message that you have won more
award points. - All callers receive another chance at a Dream
Vacation.
10Hazard Hunt Cards
- Your supervisor will issue a Hazard Hunt card,
which lists five different unsafe conditions /
practices / behaviors plus a space for other. - Every time you see one of the listed unsafe
conditions / practices / behaviors or any other
unsafe condition, tell your Supervisor.
11Hazard Hunt Cards
- On the Hazard Hunt card, keep a tally for the
month of all the month of unsafe conditions /
practices / behaviors. - At the end of the month, call the hotline to
record card totals. You will automatically be
entered in the drawing for a Dream Vacation and
may be an instant winner of more award points.
12Redeeming points for a gift
Turn to the middle of the Award of Excellence
catalog for a price list of gift items. Fill out
the order form located in the catalog OR print
out an order form online at www.incentivesonline.c
om. Mail the completed form to Award of
Excellence. Points will automatically be
deducted from personal accounts when the order is
processed. Merchandise is shipped to the
employees home by UPS and normally takes 1-2
weeks after receipt of an order.
13Can you combine points from SmartCards, You Did
It Right! card and Hazard Hunt card calls?
Yes! You can use all the points in your account
from SmartCard, You Did it Right! card and
ScoreCard calls to the Hotline. All award points
will automatically be deducted from the
employees personal account when orders are
filled!
14Treat SmartCard, You Did It Right cards and
Hazard Hunt cards like cash!
Lost or stolen cards cannot be replaced!
15You could win a Dream Vacation!
- Heres how Trips will be awarded
- The First Dream Vacation will be awarded in
October 2008 from all the trip entries received
from SmartCard, You Did It Right! Card and Hazard
Hunt calls to the hotline. - Additional drawings will be held December 2008,
March 2009, May 2009 and August 2009. - A trip winner may exchange their Dream Vacation
for 600 award points.
16You qualify to win a Dream Vacation if
- At the time of the drawing, only individuals in
overall good standing with the company are
eligible to win a trip. - Trips must be used within six months from the
time they are received. - You may choose one of the destinations listed at
www.incentivesonline.com (Click on Grand Prize
Winners).

What if I lose my You Did It Right! card before
redeeming it?
A
Were sorry, but lost cards cannot be replaced.
Once you call the hotline to redeem the card, you
may discard it.

Can I give my points to another employee?
A
No. Points and Dream Vacation certificates are
not transferable. If your employment with Lane
Furniture terminated for cause you forfeit all
unused award points and certificates.

How will I know I have won a Dream Vacation
certificate?
A
You will be notified by your supervisor that your
name was pulled in the drawing. Then, you will
be awarded a Dream Vacation Certificate.

How long does it take to receive a catalog item?
A
After mailing in your order or ordering online,
your gift will be sent by UPS to your residence.
This may take one to two weeks.
26When I complete a gift order form, how will the
Award of Excellence Gift Center know whether I
have enough points?
Q
Personal point accounts are checked when orders
are received. The points are automatically
deducted from your account when the order is
filled.
