eligible for state dollars through their local

15Option 1 2-Year Planning Process
- Previously awarded state planning grantees are
ineligible for this option. - Phase I (2009-2010) Each grantee will receive up
to 30,000 per site - Award runs from Nov 1, 2009 to June 30, 2010
- Planning Year development of community
partnerships, site, and local model. - End of year report will determine advancement
into Phase II. - Phase II (2010-2011) Depending on the of site
moving forward, each grantee will receive up to
60,000 per site. - Award runs from July 1, 2010 to June 30, 2011
- Continue planning and development, culminating in
the certification of the SBHC during spring of
2011.
16Option 2 1- Year Planning Process
- This option is available to all interested sites.
- Advance Phase (2009-2010) Grantee will receive
up to 60,000 per site. - Awards will run from Nov 1, 2009 to June 30, 2010
- Designed to cover the duration of planning and
development, culminating in the certification of
the SBHC during the spring of 2010. - Entry into Funding Formula (2010-2011) Upon
successful completion of certification in the
first year of planning, LPHA will receive funding
based on the State SBHC funding formula. - Award will run from July 1, 2010 to June 30, 2011
- Once SBHC is certified, annual awards will
continue based on each bienniums legislatively
adopted budget and compliance with State
Certification Standards.
