Objectives

• Increase cholesterol testing in the patient
  population
• Increase cholesterol testing among those with a
  diagnosis of CHD or equivalent diagnosis
• Increase the number of patients with CHD or and
  equivalent who are at goal LDL-C levels (lt100
  mg/dl)

CHD, symptomatic CVD, AAA, PVD, diabetes

• Established protocols for testing and treatment

• Preventive care flow sheet sticker
• Patient education posters on cholesterol
  testing guidelines
• Intervention time period November
