8Aetna Consumer-Directed Health Plan (CDHP)
- Consumer-directed health plans (CDHPs) are made
up of three elements - 1. A health plan
- 2. A fund or account that you can use to help
pay for qualified, out-of-pocket medical expenses
(known as a Health Savings Account or HSA) - 3. Interactive tools and information to help
you make more informed health care decisions

- Also known as an HSA
- Is an account to which UT will contribute and to
which you may elect to contribute - Tax-advantaged
- Reimburse qualified expenses tax free
- Interest bearing account
- Unused balance rolls over year to year
- Linked to a high-deductible health insurance plan
- Account is yours to keep even if you change jobs
or health plans
10How much can I contribute to my HSA?
- HSA contribution limits are 3,000 for persons
with individual coverage and 5,950 for persons
with family coverage. This includes contributions
made by UT. - The University of Toledo contributions are made
- bi-monthly.
- You may contribute by a payroll deduction,
electronic funds transfer (EFT) or with a deposit
slip. - Payroll contributions you make are prorated over
24 pay periods but can be changed throughout the
year by contacting UT HR. - Individuals over age 55 may make additional catch
up contributions - 1000 additional for 2009
11Using your HSA
- You contribute on a pre-tax basis
- You may use HSA towards your medical plan
deductible or any coinsurance or copayments. - You may use your HSA for qualified health care
expenses as allowed by the IRS. The following

17Spousal/Domestic Partner Eligibility
- Must be completed if covering a spouse/domestic
partner on Paramount Health Insurance - If spouse/domestic partner is employed part-time,
unemployed, retired, or has no benefits available
they may remain primary on coverage - If employed full-time and have health insurance
75 employer paid they may be secondary

- Available to same and opposite sex partners
starting 1/1/09. - Domestic partner registration packet available

19When Medical and Pharmacy Coverage is Effective?
- Eligibility begins upon hire date (no waiting
period). - Enrollment must occur within 30 days of hire
date. - No medical questionnaire or pre-existing
condition exclusions.
20Dental Plan
- Effective on the 1st day of the month following
employment - Coverage is provided through Delta Dental
- Routine preventative services covered in full
with no deductible
21Dental Plan Continued
- Preventative Services covered at 100
- Minor work covered at 80
- Major work covered at 50
- 50 deductible per calendar year per person 750
calendar year maximum - You may visit the dentist of your choice
22Optical Plan
- Vision Service Plan (VSP) is offered to you
- Effective the first day of the second month
following employment

- Eye examination with a 15 deductible once every
24 months - Spectacle lenses once every 24 months
- Frames covered once every 24 months
- Contact lenses may be selected instead of glasses
- Providers can be viewed at www.vsp.com.
24Group Life Insurance
- Group Life Insurance is offered through Standard
Insurance Company - Full-time employees receive coverage
- Hourly employees 10,000 term life increases by
5,000 with each 5 years of service - Salaried employees receive term life insurance in
the amount of 2 times annual salary
25Life Insurance Continued
- Accidental Death Dismemberment coverage doubles
term life insurance in force - Dependent life insurance option
- Additional life insurance option - term life or
permanent life insurance (annual enrollment)
26Flexible Spending Account
- Allows you to set aside additional money on a
pre-tax basis - May be used for out-of-pocket medical and/or
dependent care expenses - You will be reimbursed for charges incurred once
claim form is submitted - Reminder If electing Aetna medical coverage, you
are only eligible for dependent care.
27Using Your Benefits
- Paramount or Aetna will mail you an
identification card to present each time medical
services are received. - If Aetna is elected, a HSA Visa Card will also be
sent to you to access your Health Savings
Account. - Delta Dental will mail you an identification
card, however no card is required to submit claim.
28Using Your Benefits
- No card will be issued for VSP. If using a
network provider, just give the employees social
security number at the appointment. If receiving
services outside the network, send an invoice to
VSP for reimbursement. - Use your UT employee identification badge when
picking up prescriptions from the UTMC pharmacy.
Show your SXC drug card when out of town,
pharmacy is closed, etc.

35Tuition Fee Waiver Spouse/Domestic Partner and
Children
- Eligible spouse, domestic partner and dependent
children can take undergraduate classes at the
University of Toledo (after one year of service) - Benefit applies to tuition only, no credit hour
