14The baton carrier When two (or more) actors
delivers services to the same family, they choose
a baton carrier among them The baton carrier is
responsible for the co-ordination until the
involved actors agree to pass the responsibility
on to someone else The baton carrier arrange and
summons relevant actors for coordination
meetings, and leads these meetings The baton
carrier starts the relay logbook and updates it

initiated measures, the co-ordination and the
