• Patient questionnaires are forms that patients
  can fill out and submit via kp.orgs secure
  mailbox.
• They are sent as tasks in email messages. The
  information the members enter is displayed and
  stored as email encounters in the EHR.
• They can be built to use buttons, drop-down
  lists, free text fields, mandatory questions, and
  branching questions.
