Facility Submission Module

• Can be used by facilities to report
• Annual 312 reports
• 311 reports as needed
• 302 reports as needed
• Reporting fees can be paid on-line by internet
  checking, debit card or credit card
• Invoices can be printed out by the facility
• Back year reports can be filed
• Tier II reports can be printed out by the
  facility
• The Certification Statement with Fee Summary can
  be printed out by the facility
• User information, including the password can be
  updated by the facility
