An Interesting Patient
  • 55 yo male with painless swelling of the LLE of
    that has been present for months
  • Exact duration unknown, but pt described onset as
    gradual
  • Recent Doppler exam negative for DVT
  • No previous hx of DVT, OSA, Pulm/Cardiac disease
  • No current meds
  • No complaints of dyspnea, fatigue, additional
    areas of edema

Physical Exam
  • BMI 32, BP 136/86, P80, RR16, T 98.4
  • Unilateral LLE edematous
  • LLE Non-tender
  • Schaumberg purpura (brown hemosiderin deposits)
    on pretibial aspect of LLE.
  • Possible eczema / dermatitis on pretibial aspect
    of LLE

Physical Exam Continued
  • No elevated JVD
  • Pulm CTAB
  • Cardiac RRR, no MRG
  • No liver signs No ascites, spider hemangiomas,
    or jaundice

  • Elevated BMI
  • Consistent with OSA (Pulm HTN)
  • Consistent with venous insufficiency

  • Unilateral is consistent with a local cause
  • Bilateral / Generalized is consistent with a
    systemic etiology.

    Clonidine, Hydralazine, Minoxidil, Methyldopa
  • Hormones
  • Corticosteroids, Estrogen, Progesterone,

  • Nonsteroidal anti-inflammatory drugs,
    Pioglitazone, Rosiglitazone, Monoamine oxidase
    inhibitors

  • Lymphedema
  • Tumor (lymphoma, prostate cancer)
  • Surgery involving lymphatics
  • Radiation therapy
  • Infection

  • Generally acute onset
  • Usually painful
  • Elevated D-Dimer
  • Doppler used to confirm

  • Associated with varicose veins
  • Obesity

  • DDX for chronic (gt72 hr) unilateral LE edema
  • Venous insufficiency
  • Lymphedema
  • Tumor
  • Reflex sympathetic dystrophy (vasomotor
    disturbances)
  • May-Thurner Syndrome

May-Thurner Syndrome
  • Also called Iliac Vein Compression Syndrome
  • Prevalence unknown

May-Thurner Syndrome
  • The overlying right common iliac artery
    compresses the left common iliac vein against the
    lumbar vertebral bodies.
  • This has two effects on the LCIV
  • Physical entrapment
  • Intimal hypertrophy due to the chronic pulsatile
    force from the artery

  • In select cases diuretics

  • Surgical interventions
  • Usually reserved for patients that develop DVT
    secondary to May-Thurner Syndrome
  • Transluminal stenting
  • Angioplasty
  • Venous bypass surgery
  • Repositioning of the artery
