Where to find Case Log System

Log In
Your User ID and Password will be Given to you
from Residency Office.
Change your password the first time you log-in.

• Check Resident Name and Year Level
• Use drop-down menus except Competence Level
• Select Gender
• Select Panel if Continuity Clinic appt.
• Update date if needed
• Use patient number for Case id
• Use codes from billing form or use Search

• Lumbar Puncture in a 2 month old who presents
  with fever and no focus of infection
