- Cover Sheet (Packet)
- Transcript Request Form
18- What You Need In Your College Packet For The
Registrar - 1. A large (9x12 or bigger) envelope This will
hold all the papers for your packet. - 2. Cover Sheet For College Packet form This
form can be downloaded from the Guidance web
page. Fill it out and tape it to the front of
the large envelope. It tells the Registrar and
your counselor all the colleges to which you are
planning to apply, plus it has the packet due
dates. - 3. Recommendation Request form (includes Parent
Brag Sheet) These forms can be downloaded from
the Guidance web page. They help your counselor
write your letter of rec (if the college needs
one). - 4. Transcript Request form
- This form can be downloaded from the Guidance
web page. It is where you list all the colleges
(private and out-of-state public) that require
transcripts. It is also where you indicate
whether a college wants a mid-year report sent.
The form also helps you calculate how much you
owe, since you must pay to have transcripts sent.
PACKETS WILL NOT BE PROCESSED UNTIL ALL FEES ARE
PAID. - 5. A Secondary School Report form from each
college (also known as the Counselor Report or
the School Report or the Counselor Recommendation
form). You must complete the student portion of
this form. The rest of the form will be completed
by the counselor. - NOTE Not all colleges require this form, but if
there is a form, it is up to you to provide it to
the Registrar. If the college accepts the Common
App, and you want to use it, the Secondary School
Report is called the School Report. The Registrar
only needs 1 copy of this form. (She will make
any additional copies.) - 6. A Mid-Year Report form for each college
requiring that a mid-year transcript be sent. - NOTE 1 If you are using the Common App, and a
mid-year report is required, the Registrar only
needs 1 copy of the Mid-Year Report form. (She
will make any additional copies.) - NOTE 2 Not all colleges have a Mid-Year Report
form, but they still may want a mid-year
transcript. If so, you must provide the envelope
indicated in 8. - 7. A 6x9 envelope, addressed and stamped with
three first-class stamps, for each private and
out-of-state public college listed on the
Transcript Request form. The return address is
Gunns address. (Self-adhesive envelopes would be
appreciated!) - A 4x9 envelope, addressed and stamped with one
first-class stamp, for each college listed on the
Transcript Request form where you indicated a YES
in the Mid-Year Required column. The return
address is Gunns address. (Self-adhesive
envelopes would be appreciated!) - NOTE If you choose to use larger envelopes, you
must put on additional postage. To All Early
Admission Applicants When you turn in your
packet, include the above items for ALL COLLEGES
of potential interest, not just your early
admission school. - www.gunn.pausd.org / For Students / Guidance
